1. Submit your completed New Patient Form online.

2. Call the front office at 512-212-4670.

While on the phone, you will need to:

• Choose a date and time for your appointment.
• Pay a $60 deposit to secure your initial appointment.
• Confirm if appointment is in-office or telehealth.

3. Arrive at your appointment.

• If you need to reschedule, be sure to call us no later than 24 hours before your appointment to avoid a $25 fee.
