By DSM 5 diagnostic standards, an individual is not considered to be in remission from a substance use disorder until they have reached twelve weeks where they are no longer meeting criteria for an active substance use disorder. This is based on evidence that “detoxing” or “drying out” is only the first step in a comprehensive recovery plan. The strongest determining factors for prevention of early relapse involve control of mood symptoms, post-acute withdrawal and meaningful engagement with recovery support.
The ChIRP brings together an interdisciplinary team to provide clients with individualized recovery support and medication management under the supervision of a physician. Our expert staff meets to discuss client needs and progress and schedules regular team meetings with the client and their family members or other primary supports to problem solve, monitor progress, and offer complimentary referrals and services as barriers to progress are identified.
The ChIRP is designed to provide virtual recovery support for twelve weeks and can be extended whole or in part indefinitely to help you meet your recovery goals.

Week 1-4 (Intensive Post-Acute Phase – approximately 4-5 hours per week)
• Weekly Medication Management session with Nurse Practitioner.
• Initial assessment and weekly individual therapy with Licensed Professional Counselor.
• Peer Recovery Coach sessions weekly during weeks 1-4.
• Weekly Interdisciplinary Team (IDT) meeting (30 minutes) consisting of Nurse Practitioner, Individual Therapist Recovery Coach to review sessions, recovery goals, advise on medication and recovery plan.
• IDT meets with Client weekly for one hour to review prior week’s sessions, share observations, collaborate with client to affirm goals, identify strengths and answer questions from client and significant others.
• Lead Physician available as needed for consultation on all aspects of recovery plan.
• Family members, significant others encouraged to participate in IDT meetings and engage with peer recovery specialist.
Week 5-12 (Transitional Post-Acute, Early Remission – approximately 4 hours every other week)
• Bi-weekly medication management session (30min)
• Bi-weekly IDT meeting (30min)
• Bi-weekly IDT meeting with Client (30min)
• Monthly IDT meeting with Client and Lead Physician (60min)
• Weekly Peer Recovery Support check-in (15-20 min)
• Weekly individual therapy
• Total of 4 medication management, and 4 IDT meetings if engagement is longer than 12 calendar weeks due to client travel or scheduling conflicts.
